MISSING REQUIREMENTS GUIDE

hank you for applying to NSU!   At some point after you apply, you will be receiving a "missing requirements letter".  We want the admission process to be a smooth one for you, so we’ve prepared a list of different admission documents that we might need from you.   Not all items below are required from every applicant, so please refer to the letter you receive in the mail to determine which requirement(s) you are missing at that time.

 

FINAL HIGH SCHOOL TRANSCRIPT

We realize final high school transcripts are not yet available for current high school seniors.  If you are currently a high school senior attending an out of state high school,  please be sure to request your high school mail an official final transcript to NSU after you graduate.  If you are a current Louisiana high school student or graduated from a Louisiana state approved high school 2003 or afterwards, we will download a current transcript now and (if necessary) the final transcript from the state transcript system in June – please do not request any transcripts from your high school.   If you earned a GED, we will need the official test scores from the institution that awarded your GED.  If you graduated from high school prior to 2003 or from an out of state high school, please request that your official transcript be mailed to us directly from your high school.  Official transcripts are those that are delivered to the Office of Admissions in an unopened school envelope from your high school.

 

SELECTIVE SERVICE CARD

All males between the ages of 18 – 26 are required by federal law to register with Selective Service.  If and/or when you are within 30 days of your 18th birthday or older, you can register and/or print proof of registration from www.sss.gov.  If you are not yet 18 years old or within 30 days of your 18th birthday by the time you enroll for classes, we will waive the requirement. Proof of registration (printed from the website or a copy of your card) can be faxed or mailed to the Office of Admissions.

 

MMR/TD OR WAIVER - OR –MENINGOCOCCAL VACCINATION OR WAIVER

The immunization form is available at http://admissions.nsula.edu/immunization-compliance/ .  You will need to complete the top section of page 1.  The rest of page 1 is to be completed by your physician.  If your records or not available or you would like to request an exemption from the vaccination requirement, you can sign an exemption request on page 2 of the immunization form.   The request for exemption for the MMR/TD is at the top of the form on page 2, and the request for exemption for the Meningococcal is on the bottom of page 2.

 

COLLEGE TRANSCRIPT

Official transcripts are required from all former institutions attended, regardless of whether or not the credits transfer.   All transcripts must be on file and evaluated before an admission decision can be made.  Official transcripts must be delivered to the Office of Admissions in a sealed unopened envelope from each former institution to be considered official.

 

OFFICIAL ACT SCORES OR OFFICIAL SAT SCORES

Official test scores are used to determine developmental need. SOCIAL SECURITY CARD COPY

If this is requested, we either do not have a social security number on file for you, or there is a discrepancy with the number that you submitted to us.

 

APPLICATION FEE

An application fee is required for all applicants ($20 US Citizens, $30 International).  Please mail a check or money order payable to “NSU” to the Office of Admissions.
