  • Submit Proof of Immunization Compliance Form (We will also accept a copy of your personal record.  If immunizations are not up to date, your record will be considered "incomplete". Immunization records are not required to make an admission decision, but we must have them before you can enroll in classes.)

  • Submit *official High School and/or ***College Transcript(s).  The Student Transcript System (STS) will be used to obtain the official transcript for Louisiana high school graduates of years 2003 to present. Please DO NOT request your Louisiana High School trasncript if you have graduated 2003 to present.  Transfer students with less than 12 college level hours must supply high school transcript AND college transcript(s).  In compliance with FERPA (Family Educational Rights & Privacy Act) official college transcript requests must come directly from the student. • Submit **official ACT, SAT or Compass Scores.  Official test scores are those that are delivered to the Office of Admissions from ACT, SAT or the testing center where the Compass exam was given.

*An official transcript is one that is delivered to the Office of Admissions in a sealed envelope or by a secure electronic means from the previous institution(s).

**An official test score is one that is delivered to the Office of Admissions in a sealed envelope or by a secure electronic means (eScript) directly from the testing agency or center.

***Official College, University & Post-secondary transcripts are required from each previous institution.  College & University transcripts must be regionally accredited to ensure transfer of credits. Credits earned from institutions that are not regionally accredited are not required for admission. Evaluations of transfer credits and admission decisions will ONLY be made after all official transcripts have been received.

To request an official copy of your GED test scores from the LA Community & Technical College System, please go to http://www.lctcs.edu/assets/docs/Adult%20Ed/GEDRequestFormRevised102610-LCTCS.pdf. (During the 2010 Legislative session,  Senate Bill 297 by Sen.
